These products and treatments allow patients who do not want or need surgery to get rid of wrinkles, sun damage, and other skin problems without the risk and recovery time of a surgical procedure. BOTOX® Cosmetic

BOTOX® Cosmetic is a popular product for the reduction of facial wrinkles and creases, including frown lines, forehead creases, crows’ feet, and even wrinkles around the mouth. It can also be used to address thick bands in the neck.

Being in a class by itself, BOTOX® Cosmetic is not a dermal filler. It does not increase the volume of the treated area or fill in spaces beneath the skin. Instead, it consists of a purified form of a botulinum toxin which works by blocking the nerve impulses that cause your facial muscles to contract. When the muscles are relaxed, skin wrinkles do not form, leaving the treated area looking smoother and rejuvenated. BOTOX® Cosmetic treatments have been used safely and successfully for many years, allowing thousands of patients to rid themselves of undesired facial creases without having to undergo surgery.

The results of BOTOX® Cosmetic generally last between three and six months. The benefit of this is that, if the result is not satisfactory, it is not permanent and can be adjusted or discontinued. For patients who like their results, treatments can be repeated every six months to keep the appearance youthful and relaxed.

At our practice in Savannah, Georgia, we often administer BOTOX® Cosmetic along with Restylane®, Perlane®, Sculptra® Aesthetic, or other facial rejuvenation products to produce a more thoroughly pleasing overall result.

Dysport®

Attention Savannah, Hilton Head, Bluffton and Statesboro

We now have an alternative to BOTOX®. It is a product called Dysport®, made by Medicis, the makers of Restylane®. It is basically a botulinum product that relaxes muscles causing wrinkles such as the frown lines, the forehead furrows, and "crow’s feet." Time and studies will reveal its longevity compared to BOTOX®.  My early experience does no stinging from the medication, which is impressive. Also, competition historically leads to lower prices.

Restylane®

Among the most popular and frequently used dermal filler products, Restylane® is made from hyaluronic acid, a substance that naturally occurs in the human body. It restores volume and fullness to the skin and is particularly suited to filling in nasolabial folds (the creases between the nose and corners of the mouth) and fine wrinkles. It is also suitable for use as a lip enhancement product.

Restylane® is free of animal proteins, so allergic reactions are extremely unlikely and no pre-testing is required. Restylane® is quickly and easily injected into the treatment area in your physician’s office, and no recovery time is needed. Side effects may include mild bruising, swelling, or redness at the injection site. These usually disappear quickly.

The results of Restylane® treatment are immediate and generally last up to six months. Treatments can then be repeated in order to sustain a firmer, more youthful appearance. Because the results are long-lasting but temporary, the treatment can be adjusted as needed to reach a more desirable result or to account for changes in facial appearance.

Because different products have different applications, we often administer combination treatments at our Savannah, Georgia practice, using Restylane® with BOTOX® Cosmetic, for example, for maximum facial rejuvenation.

Sculptra® Aesthetic

Sculptra® Aesthetic is a synthetic injectable called Poly-L-Lactic acid. It has been used in Europe since 1999. It has primarily been used in the USA for treating lipoatrophy (facial fat loss) seen in HIV patients. Its use for cosmetic facial rejuvenation purposes is “off-label.” However, in the last two years it has been used extensively for cosmetic enhancement of the face and hands, with much success.

Faces lose volume as people age, causing sagging of the skin and a worn, aged appearance. Sculptra® Aesthetic acts as a filler, restoring volume and fullness to the face. Treatment sessions usually take about 30 minutes; three to four sessions, one every four to six weeks, are generally sufficient to achieve the desired result. Unlike dermal fillers that simply "fill in" the empty space, Sculptra® Aesthetic actually stimulates your tissues to produce more collagen and thickens the skin itself. Consequently, the results of Sculptra® Aesthetic can last for about two years or more, giving you long-lasting facial rejuvenation.

Sculptra® Aesthetic is also often used in combination with other injectables or laser skin treatments, such as the Fraxel® treatment. The effects of these combined treatments can be dramatic, while still requiring very little down time for recovery.

Perlane®

From the makers of Restylane®, Perlane® is an alternative option for patients seeking a safe and effective dermal filler. Also made with biocompatible hyaluronic acid, Perlane® does not require patients to undergo an allergy test prior to treatment. It is quickly and easily injected into the skin to add volume and smooth out wrinkles and lines. Potential side effects include redness, bruising, swelling, or tenderness at the injection site, which are usually minor and temporary.

Perlane® was developed to be thicker than Restylane®, offering more effective treatment for deeper creases and stubborn folds. Results last the same amount of time, about six months, before re-treatment is required to maintain the effects.

Juvéderm™ Injectable Gel

Manufactured by the company that makes BOTOX® Cosmetic, Juvéderm™ Injectable Gel is a dermal filler made from hyaluronic acid, the same biocompatible substance used in Restylane®. It is indicated for facial folds and wrinkles, including nasolabial folds (the creases that extend from the sides of the nose to the corners of the mouth). Two formulations of Juvéderm™ Injectable Gel are available to address both finer, softer wrinkles and deeper creases and folds.

Unlike many facial rejuvenation products, Juvéderm™ Injectable Gel has been shown to produce results that last up to a full year. The super-smooth, non-granular consistency of Juvéderm™ Injectable Gel allows it to fill in evenly and effortlessly, producing a very natural feel and appearance.

ArteFill®

An FDA-approved injectable filler used for the treatment of facial wrinkles, ArteFill® is non-absorbable, which means that it is relatively permanent compared to other dermal fillers. Like all injectable facial rejuvenation treatments, ArteFill® is administered by injection directly into the treatment area. It is commonly used to fill deep wrinkles, such as the nasolabial creases, which go from the corner of the nose to the corner of the mouth, or the marionette lines from the corner of the mouth downward. However, ArteFill® can also be used in other areas. Prior to treatment, a skin test must be performed, as ArteFill® contains bovine collagen, to which some people are allergic.

Usually one or two treatments of ArteFill® are all that is needed to promote long-lasting facial rejuvenation. In some cases, a third may be desired for touch-up. Like any other cosmetic procedure, ArteFill® does not stop the aging process. As with all injectable fillers, one may experience some swelling or bruising after treatment, which will disappear within a couple of days.

Evolence®

Evolence® is a new injectable product for diminishing facial wrinkles and folds, such as the nasolabial crease (the crease between the nose and corners of the mouth), also called the smile lines. As we age, certain areas of the face de-volumize, and we lose collagen. Although Evolence® is a natural collagen dermal filler, no skin test is necessary prior to injections. The advantages of Evolence® are minimal down-time and recovery as well as minimal bruising, swelling and discomfort. Evolence® has been clinically proven to last up to a year.

Although new to the US, Evolence® has been used extensively in Canada, Europe, and Israel for several years.

Lipodissolve™ (Injection Lipolysis)

The most common procedure performed by plastic surgeons for the removal of excess fat is liposuction. The areas most frequently treated with liposuction are the abdomen, thighs, sides, back, arms, and neck. This procedure often requires general anesthesia and a significant amount of recovery time, depending on the degree of fat removal.

Lipodissolve™, also called injection lipolysis, is a new technique that offers fat removal without the hassle of surgery. Like liposuction, Lipodissolve™ is not indicated for people who are overweight or those with excessive loose skin. It is simply a method of removing small deposits of fat that are non-responsive to dieting and exercise.

Mostly performed by European plastic surgeons, Lipodissolve™ has been effective for tens of thousands of patients. The procedure involves the injection of a product known as phosphatidylcholine (PPC) into the fat just under the skin. The PPC dissolves the fat, causing it to break down and be eliminated from the body. We generally expect to see some shrinkage of skin also. Because PPC is made from a soybean extract, Lipodissolve™ is not indicated for those with soybean allergies.

Currently, PPC has not been approved by the FDA for use in Lipodissolve™ treatments. However, the research that has been done and studies that are still being conducted show promise for this cutting-edge treatment. At Finger and Associates Plastic Surgery Center, we feel strongly that Lipodissolve™ should only be administered by a qualified physician, such as a plastic surgeon, or under the supervision of such a physician. We take every precaution to maximize patient safety.

A Lipodissolve™ treatment takes approximately 30 minutes and involves numerous injections of PPC. The injections are administered with a very small needle and placed at half-inch intervals into the area of the unwanted fat. No anesthesia is needed, although a topical anesthetic cream is generally applied to minimize the discomfort of the injections. Some people also request a mild tranquilizer. Immediately after the injections, we focus a very powerful ultrasound machine over the area to aid in product distribution. We suggest repeating this stage of the procedure two or three times over the next week.

After receiving the Lipodissolve™ injections, one can expect swelling for a week or so. The area will be sore, tender, and pink. Most people who undergo Lipodissolve™ go right back to work, although they may be somewhat uncomfortable. It is necessary to drink lots of water before and after each Lipodissolve™ treatment and avoid overheating for a few days by refraining from working out or taking a steaming hot shower.

It may take several Lipodissolve™ treatments for optimal results to appear, but once the desired outcome has been achieved, repeat treatments should not be necessary.

Non-Surgical Face Lifts

Radiesse®

Radiesse® is a dermal filler that can be used to enlarge lips and fill in folds and wrinkles on the face. It consists of calcium hydroxylapetite, a normal constituent of bones, suspended in a biocompatible gel. Like Restylane®, it requires no pre-testing for allergies. The results of treatment with Radiesse® can last up to two years. Another benefit of Radiesse® is its effectiveness as a treatment for acne scars.
